Wellston School District
New Student Enrollment Requirements
Children who live in the attendance area of the Wellston School District are eligible to attend schools within the district. Parents may enroll their children at the Accountability Office, which is located at the district administration office at 6574 St. Louis Ave., St. Louis, MO 63121. The Accountability Office is open for student enrollment Monday through Friday from 9:00 a.m. to 3:00 p.m. We are closed for legal holidays. For better service, please call (314) 290-7941 to schedule an appointment.
As parent/guardian, you will need the following items to enroll your child(ren) in the Wellston School District:
- Proof of Residency (occupancy permit updated within the past 90 days, mortgage, lease agreement, current unpaid bill with your name/address). If you do not have proof of residency, you may have to submit a residency waiver request; or,
- Affidavit of Residency – if living in someone else’s home. PLUS: Current mail (no ads) in the name of the person registering the student.
- Student’s original birth certificate (with raised seal, state issued)
- Student’s original social security card*
- Up-to-Date Immunization record (complete from infancy, required by State).
- Verification of physical exam (within the last 24 months)
- Special Medical Needs, Emergency or Asthma Action Plan – listing medications
- Last report card for elementary student or unofficial transcript for high school.
- Withdrawal form from previous school (if registering during the school year).
- Discipline report (in sealed envelope will expedite enrollment).
- IEP and Evaluation (if receiving special education services or if being serviced by Special School District) or 504 Plan
- Proof of Guardianship (Court Appointed Documents, Divorce Decree, DFS Placement Papers, Foster Care)
- Photo ID – Drivers License of parent or guardian enrolling student.
*Disclosure of the social security number is voluntary; however, this information is requested in order to offer or provide school-based health, counseling, or other educationally-related services and to coordinate services between agencies, when needed.
*A student’s DCN number may be the same as their Medicaid/MOHealthNet number and is required to process the Free and Reduced Lunch Application and/or for participation in the school-based health screenings, dental clinic, etc.
The Wellston School District can only enroll students that provide all the above listed documents.
